Nominate an Applicant or Apply for an Award

Applicants must be between the ages of 14 and 21

To apply for an Award, please open the attached application and save it to your computer.  Print, complete and mail the application to the address on the form. 

To nominate someone you think deserves this award, please complete as much of the application as you can, including one letter of recommendation.  Mail it to the address on the application and the foundation will contact the individual to complete the process.
